  • Hospice Residence Nurse Manager

    Title: 

    Hospice Residence Nurse Manager


    ($37-$41)


    Hospice of Jefferson County is seeking a full time Residence Nurse Manger. They would provide professional nursing care to patients, and supervises the care provided by direct care staff (LPNs and Aides) by performing the following duties.

    Duties and Responsibilities:

    Include the following. Other duties may be assigned.


    • May meet prospective patients and their families to determine interest and appropriateness for placement in the Residence.
    • Establishes and monitors plans of care, in coordination with the Hospice Interdisciplinary Team.
    • Educates patients and families regarding the Interdisciplinary Plan of Care.
    • Ensures that patient assessments are accurate, complete and timely.
    • Works with the Social Worker and Bereavement Coordinator, providing emotional support to patients and their families during the dying and grieving process.
    • Trains and supervises direct patient care staff to include professional development, mentoring, and coaching.
    • Ensures efficient and effective 24 hour, seven days a week schedule of care for patients/families.
    • Works with the Director of Patient Care and assist with recruiting staff to work in the Residence.
    • Develops and implements programs and training for staff development and orientation.
    • Ensures/provides complete comprehensive orientation for new nursing employees.
    • Completes performance appraisals on a timely basis.
    • Maintains confidentiality, inclusive of HIPAA regulations, of all pertinent patient care information.
    • Adheres to New York State mandated reporting requirements.
    • Ensures compliance with established policies and procedures for all areas of operations to include but not limited to medication administration, inventory control, food service, housekeeping, and regulatory agencies.
    • Assists Director of Patient Care in staff education, scheduling and Quality Assessment /Performance Improvement (QAPI).
    • Is an active participant in the Leadership Team, infection control, Safety, Safe Patient Handling and QAPI Committees.
    • Performs other related duties as assigned.
  • LPN (Home Care/Residence) Part-Time Day Shift (7:00am - 3:00pm) & Part-Time (Night Shift 11:00pm - 7:00am) - Hospice Residence

    Hospice of Jefferson County is seeking a per diem Home Care/Residence LPN. You will be part of an interdisciplinary team focused on palliative care and symptom management in the patient's home. A valid NY State LPN License and Driver's License is required. Competitive pay based on work history and mileage reimbursement.


    In the Hospice Residence, shift differential will apply for evening and night shifts. Rotating weekends with a safe patient ratio of 6 to 1.


    Residence Job Duties below:


    1. Provides nursing care according to the established plan of care and under the direction and supervision of the RN.
    2. Provides emotional support to patient and family.
    3. Maintains confidentiality, inclusive of HIPAA regulations, of all pertinent patient care information.
    4. Prepares light meals and/or snacks for patients as needed.
    5. Performs light housekeeping in patient's immediate environment.
    6. Adheres to New York State mandated reporting requirements.
    7. Participates in Quality Assurance Improvement projects.
    8. Works non-judgmentally with patients and families of many cultures, religious, socioeconomic backgrounds, and varied lifestyles.
    9. Adheres to agency policies and accepted nursing practices and regulations.

  • RN Case Manager

    Range 37-40 per hour


    Hospice of Jefferson County is seeking an immediate full time RN Case Manager interested in making a difference for those at the end of life. You will be part of an interdisciplinary team focused on palliative care and symptom management in the home or nursing home setting. A valid NY State RN License and COVID vaccination are required. Competitive pay based on work experience and mileage reimbursement.


    Job Duties include: 

    The RN Case Manager will manage each patient’s care and be responsible for the overall plan of care and the outcome of that care, including care delivered by other nursing staff (LPNs) who will be directed / supervised by the RN. Overall responsibilities include planning, providing and coordinating quality end of life care.


    Required Skills: 

    Able to work independently, and complete documentation accurately and in a timely fashion. Valid driver’s license. Must be willing to travel throughout Jefferson County if required.

  • Hospice Navigator

    Range 24-27 per hour


    Job Overview

    We are seeking a compassionate and dedicated Hospice Navigator to join our team. The Hospice Navigator will play a crucial role in guiding patients and their families through the hospice care process, ensuring they receive the support and resources they need during this sensitive time. This position requires strong interpersonal skills, empathy, and the ability to work collaboratively with healthcare professionals.


    Duties

    1. Develops and maintains linkages with community healthcare providers to establish and support new referral sources.
    2. Develops, implements and evaluates quarterly and annual plans to achieve agency growth goals.
    3. Implements agreed upon strategies and actions to achieve goals.
    4. Serves as the primary point of contact for incoming referrals and admissions­ related questions.
    5. Provide compassionate education to patients, families, and caregivers regarding hospice philosophy, services, and expectations.
    6. Actively identifies referrals from physicians, discharge planners, hospitalists, hospital/community social workers, hospital palliative care program, certified home care agencies and patient/families.
    7. Maintains an active and in person relationship with providers and agency personnel to identify referrals at the earliest possible time.
    8. Meets with potential patients and families to explain hospice program. Ensures understanding and desire for admission.
    9. Verify referral information and demographics, obtains clinical information, and communicates payer eligibility with billing.
    10. Works with the Resource Nurse to assure timely coordination and completion of the admission process.
    11. Logs daily call activity and documents plans for follow up activities with the referral sources and/or patients.
    12. Prepares monthly tracking reports on referral sources; identifies key marketplace changes related to providers.
    13. Records all activity in the EMR throughout the referral process.
    14. Maintains confidentiality, inclusive of HIPAA regulations, of all pertinent patient care information.
    15. Adheres to New York State mandated reporting requirements.
    16. Performs other related duties as assigned.

    Qualifications

    • Social Work or Nursing experience preferred
    • Previous experience in hospice care or palliative care is highly desirable.
    • Strong communication skills with the ability to convey complex information clearly and compassionately.
    • Ability to work collaboratively within a multidisciplinary team environment.
