CLINICAL POSITIONS

  • Part-Time to Home Care RN Casual

    Job Duties Include: 

    Part-Time to Home Care RN Casual will manage each patient’s care and be responsible for the overall plan of care and the outcome of that care, including care delivered by other nursing staff (LPNs) who will be directed / supervised by the RN. Overall responsibilities include planning, providing and coordinating quality end of life care.


    Required Skills: 

    Able to work independently, and complete documentation accurately and in a timely fashion. Valid driver’s license. Must be willing to travel throughout Jefferson County if required.


    Job Type: 

    Part-Time


    Pay: 

    $37.00 - $39.00 per hour

  • Per Diem LPN (Home Care/Residence) Part-Time (Night Shift 11:00pm - 7:00am) - Hospice Residence

    Hospice of Jefferson County is seeking a per diem Home Care/Residence LPN. You will be part of an interdisciplinary team focused on palliative care and symptom management in the patient's home. A valid NY State LPN License and Driver's License is required. Competitive pay based on work history and mileage reimbursement.


    In the Hospice Residence, shift differential will apply for evening and night shifts. Rotating weekends with a safe patient ratio of 6 to 1.


    Residence Job Duties below:


    1. Provides nursing care according to the established plan of care and under the direction and supervision of the RN.
    2. Provides emotional support to patient and family.
    3. Maintains confidentiality, inclusive of HIPAA regulations, of all pertinent patient care information.
    4. Prepares light meals and/or snacks for patients as needed.
    5. Performs light housekeeping in patient's immediate environment.
    6. Adheres to New York State mandated reporting requirements.
    7. Participates in Quality Assurance Improvement projects.
    8. Works non-judgmentally with patients and families of many cultures, religious, socioeconomic backgrounds, and varied lifestyles.
    9. Adheres to agency policies and accepted nursing practices and regulations.

  • Bereavement Coordinator

    At Hospice of Jefferson County we are dedicated to providing holistic, compassionate end-of-life care to patients and their families. Our interdisciplinary team works together to support not only the physical and emotional well-being of our patients but also the ongoing support of their loved ones after a loss. We are currently seeking a compassionate and organized Bereavement Coordinator to join our hospice team.



    Job Summary:


    The Bereavement Coordinator is responsible for the planning, coordination, and implementation of bereavement services and coordinating spiritual care for families and caregivers of hospice patients. This includes individual support, support groups, memorial services, and community education. The ideal candidate has strong interpersonal skills, clinical knowledge of grief and loss, and a genuine desire to support individuals during some of the most difficult moments in their lives.



    Key Responsibilities:


    • Develop and implement bereavement care plans for families and caregivers.
    • Provide one-on-one grief counseling and support as needed.
    • Facilitate grief support groups and community memorial services.
    • Maintain accurate records and documentation in accordance with hospice standards and regulatory requirements.
    • Conduct risk assessments to identify individuals at risk for complicated grief and refer to appropriate mental health professionals when necessary.
    • Provide education and support to hospice staff and community groups regarding grief and loss issues.
    • Collaborate with the interdisciplinary hospice team to ensure seamless care for families throughout the bereavement period (minimum 13 months post-loss).
    • Assist in developing and distributing bereavement-related materials and resources.
    • Assist in social work duties as needed. 

    Qualifications:


    • Master’s Degree with valid NYS license. 
    • Minimum of 2 years of experience in grief counseling, hospice, palliative care, or related field.
    • Knowledge of grief theory and bereavement best practices.
    • Strong communication, organization, and documentation skills.
    • Ability to work independently as well as part of a multidisciplinary team.
    • Comfortable working with diverse populations and cultural traditions.
    • Valid driver's license and reliable transportation (if home visits are required).

    Preferred:


    • Licensed in social work, LMSW
