Advanced Care Planning

Advance care planning is about planning for the “what ifs” that may occur across the entire lifespan. —Joanne Lynn, MD

Though we don’t like to think about it, an accident or change in health can happen to anyone. That’s why it’s important for everyone, regardless of age or health, to have a plan that documents our health care wishes should we become unable to make important treatment decisions to communicate our choices. The best time to share these wishes is when we are healthy, thinking clearly, and able to participate in the conversation.

Five Wishes

Five Wishes takes the guessing out of caring. It is a legal document that guides you through planning before a health crisis.

Five Wishes is changing the way we talk about advance care planning. More than just an advance directive, Five Wishes brings a holistic approach to a living will by including an individual’s care and comfort choices. It provides a means to legally document your choices for medical treatment, comfort, and care wishes.

Completing the Five Wishes is a gift to your family, friends, and your doctor because it keeps them out of the difficult position of having to guess what kind of treatment you want or don’t want, and helps provide them with a peace of mind—knowing that they honored your choices.
