Services provided by Hospice of Jefferson County
Hospice is a caregiving team of professionals and volunteers working together to serve terminally ill patients and their families. The medical, psychological and spiritual support, primarily given in the home, enables families to remain together in peace, comfort and dignity. Hospice provides a comprehensive continuum of services, including nursing care; medical social services; physician services; counseling; short-term in-patient care; medical appliances and supplies; and physical, occupational and speech therapies.
Team of professionals and serving each family
Hospice care relies on the combined knowledge and skills of a full team of professionals called the interdisciplinary team. This team usually includes a physician, nurse, therapist, counselor, and social worker. Together these professionals with the help of volunteers develop an individualized plan of care. Twenty-four hour on-call availability. The hospice team is on-call 24 hours a day, 7 days a week.
Patient-family focused. Hospice care differs from other forms of health care in that it focuses on the family and not just the terminally ill patient. The in-home centered philosophy helps preserve one of our country’s most important social values; keeping families together.
Primary goal to relieve pain
Hospice staff is trained specifically on palliative care or “symptom control†and “comfort care.†Hospice helps the patient live his or her days as peacefully as possible.
Acceptance regardless of financial resources
Although hospice services are covered under Medicare, Medicaid, most private insurance groups and HMO’s, community contributions,
memorial donations and foundation gifts make it possible for hospice to provide services regardless of patients’ insurance or ability to pay.
